Bullying - Let's talk about the 'Elephant in the Room'

Bullying is at an all time high and very much the taboo subject. How should it be managed?An example : Group of staff come forward about bullying from colleagues. They give numerous examples & I was appalled. I escalated to management. Numerous meeting's occurred. Many tears shed, some went off sick.
Solution - bring in external company to ‘ Mediate’.
Staff all had to sign NDA
Staff interviewed by external mediator
Staff wrote statements
Staff then expected to sit face to face with perpetrators & tell them how it made them feel. Subsequently most staff left
IS this accountability and responsibility for perpetrators?
